What a Support Coordinator does
In New Jersey's DDD system, a Support Coordinator helps eligible individuals develop their Individualized Service Plan (ISP), connect with approved providers, and monitor that services are delivered well.

Once the ISP identifies the needed services, approved providers like Harmony Support deliver them — Gemeenschapsgerichte Ondersteuning, Individuele Ondersteuning, Respite, Vervoer, and Community Inclusie.

We communicate directly with your Support Coordinator and family, deliver the services listed in the ISP, and keep everyone updated on progress and outcomes.
