Community-Based Support
Our services help individuals achieve the goals set in their Service Plans — providing guidance for community-based activities, plus training and supervision to develop essential self-care tasks, social skills, and activities of daily living, so participants build the skills to thrive in all areas of life.
Respite Care
Compassionate respite that gives primary caregivers temporary relief while ensuring the continued well-being of their loved one. Our trained professionals provide in-home or out-of-home care with personalized support — daily living tasks, social activities, and personal care, all tailored to the individual.
Individual Support
Personalized one-on-one assistance that helps individuals live as independently as possible: activities of daily living (ADLs), customized meal planning and preparation, household management and safety awareness, and support with appointments and medication reminders.
Community Inclusion Services
Helping individuals participate fully in their communities — engaging in local events, volunteer opportunities, and social activities; building meaningful relationships and social skills; exploring hobbies in inclusive settings; and enhancing self-confidence and independence.
Transportation Services
Safe, reliable transportation to medical and therapy appointments, employment and vocational programs, community activities, social events and day programs, plus shopping and essential errands.
