How to Apply for NJ DDD Services: A Family's Guide

What is NJ DDD?

New Jersey's Division of Developmental Disabilities (DDD) funds support services for eligible adults (age 21+) with intellectual and developmental disabilities — helping them live, work, and take part in their communities.

Step 1: Confirm eligibility

Eligibility generally requires a qualifying developmental disability that began before age 22, New Jersey residency, Medicaid eligibility, and being 21 or older. Determination is made by DDD.

Step 2: Apply and enroll in Medicaid

Because most DDD services are funded through Medicaid, enrolling in NJ Medicaid is a key early step. Keep documentation of diagnosis and income handy.

Step 3: Get a Support Coordination agency

Once eligible, you choose a Support Coordination agency. Your Support Coordinator helps build an Individualized Service Plan (ISP) that lists the services and goals for your loved one.

Step 4: Choose your providers

With an ISP in place, you select approved providers — like Harmony Support — for services such as Community-Based Support, Individual Support, Respite, Transportation, and Community Inclusion.
